چکیده :

Context: The 7th package of health reform in Iran is concerned with the “promotion of natural childbirth”. It has been focusing on reducing caesarean section and promoting normal vaginal delivery as broadly as possible. Aim: This study evaluated the implementation of this plan in public and private hospitals of Isfahan Province in 2014. Settings and Design: This is an applied descriptive, comparative study. The population of this study contained all public and private hospitals in Isfahan province covered by the Health Reform Plan. It included 22 public and 6 private hospitals Methods and Material: The data were collected by Isfahan University of Medical Sciences. In this study the data collected in 6 months before and after the implementation of the plan have been analyzed. Statistical analysis used: SPSS software version 16.0 (SPSS Inc, Chicago, IL, USA) and paired T-test have been employed for data analysis. Results: It was demonstrated that the increasing rate of normal vaginal delivery and decreasing rate of caesarean section in public hospitals after the implementation of the plan were significant. On the other hand the increasing rate of caesarean section in private hospitals was significant (P-value < 0.05). Conclusions: Although this scheme has achieved its targets in public hospitals of Isfahan province, it hasn’t met its objectives in private hospitals. Caesarean section operations were shifted from public hospitals to private ones, which is in conflict with the objectives of the Health Reform Plan.
