چکیده :

Background: Tuberculosis is a global problem and its occurrence in each region is influenced by different biological and environmental factors. This study aimed to investigate the influence of environmental and climatic factors on the transmission and occurrence of tuberculosis in southwestern Iran. Materials and methods: This descriptive-analytical study was performed using a complete enumeration method on all patients with tuberculosis in Kohgiluyeh and Boyerahmad Province in southwestern Iran. The patients' spatial data were collected over 11 years from 2006 to 2016 and included in the political division layer (village/city). The iso-thermal, iso-humid, and iso-hydral layers were constructed based on interpolation models using the data from the Meteorological Department of the province. The slope layer was constructed based on the digital elevation model. Finally, the effect of temperature, rainfall, humidity, altitude, slope and land cover on the occurrence of tuberculosis in this region was evaluated using the logistic regression models. Results: 336 cases of tuberculosis were reported during this 11-year period in the province with the highest and lowest prevalence rates of 0.1% and 0.0005%, respectively. Based on the results of the univariate logistic regression model, urban land cover, irrigated and rainfed fields and gardens, and moisture had a positive impact and slope, elevation, and minimum annual temperature had a negative impact on disease occurrence. Besides, in the multivariate model, only urban cover, slope, and elevation were found as the main determinants affecting the occurrence of tuberculosis. Conclusion: According to the results of this study, the most important predictors of TB in southwestern Iran are the urban cover, slope, and elevation. High-risk areas were urban regions located in lower slopes and elevations. Besides, farming regions and areas with lower humidity and lower minimum temperatures should also be considered. These areas need to be focused more in terms of budget allocation and ongoing monitoring to control the disease.
